NON-PERSONALIZED FINANCIAL ESTIMATE
Upon a patient’s request, the ASC and health care providers can provide a non-personalized estimate of charges and other information prior to the service; including patients with no insurance. Please note that the payment ranges are an estimated cost that may be incurred and your actual cost may vary based on actual services rendered.
PAYMENTS NEED AT TIME OF SURGERY
If additional insurance information is needed or if you will need to make a payment at the time of your surgery, one of our business office team members will contact you prior to your procedure. Co-payments and Co-Insurance will be collected at the time of service. Depending on your deductible balance, you may owe additional fees and will receive a bill if indicated. If your account is not paid in full (unless prior plans have been made) within 90 days, you may be turned over to collections.
BUDGET AGREEMENT
To assist a patient in setting up a payment plan for out-of-pocket costs such as co-insurance, deductibles, etc. please contact: Surgery Center at Sacred Heart Medical Park at (850) 269-0301. You may also apply for CareCredit convenient financing options by visiting www.carecredit.com or by calling 800-365-8295 prior to your day of surgery.
PAYMENT METHODS ACCEPTED
MasterCard, VISA, American Express, Discover and Care Credit. Cash, Cashier’s check, money orders and personal checks. If paying by personal check, you are authorizing The Surgery Center at Sacred Heart Medical Park, to electronically debit your account for the amount of the check plus processing fee (up to state legal limits) if your check is dishonored.
COSMETIC PROCEDURES
Fees will be due on or before the date of surgery.
PRIVATE PAY
Estimated charges will be due on the date of service.
FLORIDA MEDICAID
Effective January 1, 2022 we are not participating providers for Florida Medicaid,. As noncontracted providers, you will be responsible for any payments (if primary) or coinsurance amount(s) as deemed necessary by your primary insurance.
WORKER’S COMPENSATION
If benefits are authorized in advance, no payment is due at time of service.
PAYMENTS MADE TO YOU
If any payment is made directly to you for services billed by the facility, you are obligated to promptly submit same to Surgery Center at Sacred Heart Medical Park.
Other Providers
Services may be provided by the ASC as well as by other health care providers who may separately bill the patient and may or may not participate with the same health insurers or health maintenance organizations as the facility. You will receive separate bills from Surgery Center at Sacred Heart Medical Park, your physician, and your anesthesiologist. You may also receive a bill for any laboratory, pathology, diagnostic services, or implants that you receive. Patients should contact each health care provider who will provide services in the ASC to determine their coverage and participation with the health care plan.
